About Communications & Room Requests
The Communications & Room Request form is the only approved mechanism to request assistance from the Grace Communications department and/or to request the use of any of Grace’s facilities from the Grace Service Ministries department. Please fill out all fields applicable in your request. If you’re having difficulty filling out this form, contact us and a staff member will assist you.
Guidelines
Submissions must adhere to these guidelines. By submitting this form, you affirm that the information entered conforms to these rules and expectations, and that you understand the expectations pertaining to your use of any facilities at Grace Polaris Church. Failure to comply with these rules and expectations may result in the denial of your request and/or additional fees.
The Service Ministries department determines event placements. While you have the ability to specify specific space(s) in your request, gatherings will be scheduled in the most appropriate space(s) as determined by Grace Polaris Church’s Service Ministries department. The Service Ministries department reserves the right to determine all event placements based on numerous factors, including event size, setup needs, staff availability, and other considerations.
Please note that Grace Polaris Church does not actively manage room assignments for the Family Center, Discipleship Center, or athletic facilities. Please contact Worthington Christian School to make such arrangements.
If your event requires the storage or preparation of food, then your party must adhere to the Ministry Center Kitchen Use Guidelines. Adherence to these guidelines is essential for Grace Polaris Church to maintain its license to operate its commercial kitchen. Failure to comply with these rules and expectations may result in denial of future requests and/or additional fees.
For requests which only involve Communications Department assistance, simply select “Offsite” or “Virtual” on the first page.
Pricing
Payment may be required for your event. Once your request has been submitted, a Grace staff member will contact you to form an estimate for the cost of hosting your event.
If/when your room request is approved, a Grace staff member will email an estimate specific for your event. 50% of the total payment is due to Grace Polaris Church immediately upon signing the estimate agreement, and the balance will be due no later than 30 days prior to the event date. All payments must be remitted via check with “Facilities Fees” in the memo line and mailed to: Grace Polaris Church, Attn: Facilities Department, PO Box 1650, Westerville, Ohio 43086.
You may cancel your event no later than 30 days prior to the event date for a full refund, while a refund is not guaranteed if the event is canceled within that 30-day period. Additional fees may be incurred for any damage to the space(s) utilized during your event, or if any spaces are left in disarray.
